Hematochezia is the passage of fresh, bright red or maroon blood from the rectum. The term appears in clinical notes, discharge summaries, and referral letters, and it is often the first place a person encounters a name for something they have been quietly worried about for weeks. It is a description of a finding rather than a diagnosis — it tells you what is being observed, not why.

The single most useful thing to understand about it is that color approximates distance. Blood that appears bright red has generally travelled a short way and had little contact with digestive enzymes, pointing toward a source in the anorectum or distal colon. Blood that has been in the gastrointestinal tract longer is progressively degraded, turning maroon and then, if it has come from the stomach or duodenum, black and tarry — a distinct finding called melena. That gradient is the reason clinicians ask about color before almost anything else. It is a rough guide rather than a rule, and one important exception is covered below, but as a starting framework it holds.

What Hematochezia Is — and What It Is Not

The word combines the Greek roots for blood and for stool, and in clinical usage it means visible fresh blood passed rectally, whether mixed through the stool, coating its surface, dripping into the bowl, or appearing only on the paper. You may also see it abbreviated BRBPR, for bright red blood per rectum, which describes the same finding in plainer language.

What it is not is a diagnosis, and this distinction matters more than it may appear. A great many conditions produce hematochezia, they range from trivial to serious, and they are not distinguishable from the appearance of the blood alone. Two people can describe visually identical findings and have entirely different underlying causes. This is why evaluation is directed at establishing the source rather than at the bleeding itself.

It is also worth separating hematochezia from things that mimic it. Beetroot, red food coloring, and certain medications can turn stool red without any blood being present at all. Bleeding from the vagina or from the urinary tract can be misattributed to the bowel, particularly when it is noticed on paper or in the bowl rather than seen directly. And a small streak of blood on the outside of an otherwise normal stool carries different implications from blood mixed evenly through it, because surface blood suggests the stool acquired it on the way out.

Hematochezia, Melena, and Occult Bleeding

Gastrointestinal bleeding is conventionally described in three categories based on how the blood appears by the time it is seen. Understanding which category a finding falls into narrows the likely source considerably before any test is performed.

  • Hematochezia: Fresh red or maroon blood passed rectally. Usually indicates a source in the lower gastrointestinal tract — the anorectum, colon, or occasionally the distal small bowel.
  • Melena: Black, tarry, strikingly foul-smelling stool. The color comes from blood degraded by stomach acid and enzymes, which generally indicates an upper gastrointestinal source such as the stomach or duodenum.
  • Occult bleeding: Blood present in quantities too small to change the appearance of the stool, detectable only by laboratory testing. It is often found during screening or discovered while investigating unexplained iron-deficiency anemia.

The exception to the color rule is clinically important and worth stating explicitly. A brisk, high-volume bleed from an upper gastrointestinal source can move through the tract too quickly to be degraded, and it will then present as hematochezia rather than melena. What distinguishes this scenario is not the appearance of the blood but the state of the person: rapid heart rate, low blood pressure, pallor, sweating, dizziness, or fainting. Fresh rectal bleeding accompanied by any of those signs is treated as a potentially major upper gastrointestinal hemorrhage until proven otherwise, and it is a medical emergency.

Occult bleeding deserves a brief note as well, because it is the category most likely to be overlooked. Bleeding slow enough to leave no visible trace can still, over months, produce meaningful iron-deficiency anemia. That is why unexplained anemia is itself an indication for gastrointestinal investigation even when the person has never seen blood at all.

Common Causes of Hematochezia

The causes below are grouped by anatomical origin, which is roughly how a clinician thinks through the problem. Frequency varies substantially with age: anorectal causes dominate in younger adults, while diverticular and vascular causes become progressively more common with age.

Anorectal causes

  • Hemorrhoids: Enlarged vascular cushions in the anal canal, and one of the most common causes of visible rectal bleeding overall. Internal hemorrhoids typically produce painless bright red bleeding, often seen on the paper or coating the stool rather than mixed through it.
  • Anal fissure: A small tear in the lining of the anal canal, usually caused by passage of a hard stool. It characteristically produces sharp pain during and after a bowel movement, with a modest amount of bright red blood.
  • Proctitis: Inflammation of the rectal lining, which may result from inflammatory bowel disease, infection, or previous radiation therapy to the pelvis. It often produces bleeding accompanied by mucus and a persistent urge to pass stool.

Colonic causes

  • Diverticular bleeding: Bleeding from small outpouchings in the colon wall, which becomes more common with age. It is classically painless, can be brisk, and often stops on its own, though it may recur.
  • Angiodysplasia: Fragile, abnormally formed small blood vessels in the bowel wall. It is a recognized cause of recurrent or intermittent bleeding, particularly in older adults.
  • Inflammatory bowel disease: Crohn disease and ulcerative colitis both cause inflammation and ulceration of the bowel lining. Bleeding is typically accompanied by diarrhea, urgency, abdominal pain, and sometimes weight loss.
  • Colorectal polyps and cancer: Growths in the colon or rectum can bleed, sometimes intermittently and often without pain. This possibility is one of the main reasons persistent or unexplained rectal bleeding warrants evaluation rather than observation, and it is the reason screening exists — the large majority of hematochezia does not turn out to be this, but it is not something to establish by assumption.
  • Infectious colitis: Certain bacterial infections invade the bowel lining and produce bloody diarrhea, usually with fever, cramping, and an abrupt onset.
  • Ischemic colitis: Reduced blood flow to a segment of colon, typically presenting with sudden abdominal pain followed by bloody stool, most often in older adults.

Other contributors

  • Anticoagulant and antiplatelet medication: Blood thinners do not themselves create a bleeding source, but they can make a minor one bleed considerably more and for longer. Bleeding while taking one of these medications warrants prompt assessment rather than watchful waiting.
  • Brisk upper gastrointestinal bleeding: As described above, a rapid bleed from the stomach or duodenum can present as fresh rectal blood accompanied by signs of circulatory compromise.

Reading that list end to end can be alarming, which is worth addressing directly rather than leaving to sit. In practice, the anorectal causes at the top account for the substantial majority of cases presenting to primary care, and hemorrhoids and fissures together account for most of those. This is not a reason to assume the worst, but it is a reason to have persistent or unexplained bleeding assessed rather than self-diagnosed, because the appearance of the blood genuinely does not distinguish between the entries on this list.

The pattern of the bleeding, however, does narrow things usefully. When Hematochezia Requires Urgent Care

Most rectal bleeding is not an emergency. The findings below are the ones that change that assessment, and they are worth knowing precisely because the volume of blood alone is a poor guide — a small amount dispersed in toilet water looks dramatic, while a slow ongoing bleed may look like very little.

Emergency — call 911 or go to an emergency department:

  • Large-volume bleeding, or bleeding that continues without slowing
  • Dizziness, fainting, confusion, racing heart, or difficulty staying upright
  • Pallor, cold clammy skin, or sweating alongside the bleeding
  • Severe abdominal pain with bloody stool
  • Black tarry stool with lightheadedness, or vomiting blood or material resembling coffee grounds

Same-day or next-day assessment:

  • Bleeding while taking an anticoagulant or antiplatelet medication, or with a known bleeding disorder
  • Bloody diarrhea with fever
  • Bleeding accompanied by unexplained weight loss or a sustained change in bowel habit
  • A first episode of rectal bleeding at age 45 or older

The emergency tier is defined less by the blood than by what the circulation is doing. Signs of volume loss — dizziness, a racing heart, pallor, fainting — indicate that bleeding is outpacing the body’s ability to compensate, and that is true regardless of how much blood was actually seen. The middle tier exists because anticoagulation, systemic symptoms, and age all raise the probability that something beyond a simple anorectal source is responsible.

How the Source Is Identified

Evaluation is a process of localization. Because the appearance of the blood cannot establish the cause, the sequence moves from the least invasive assessments toward direct visualisation, guided at each step by what has been found so far.

  • History and examination: Color, timing relative to bowel movements, associated pain, bowel habit, medications, and family history all narrow the differential substantially before any test is ordered.
  • Anorectal examination: Direct inspection and digital examination, sometimes with anoscopy, allows the anal canal and distal rectum to be assessed and identifies fissures, external findings, and many internal hemorrhoids.
  • Blood tests: A full blood count establishes whether bleeding has produced anemia and gives a sense of chronicity; clotting studies are relevant when anticoagulation or a bleeding disorder is in question.
  • Colonoscopy: Direct visualisation of the entire colon and rectum. It remains the definitive investigation for lower gastrointestinal bleeding because it both identifies the source and allows biopsy or treatment during the same procedure.
  • Upper endoscopy: Performed when an upper gastrointestinal source is suspected, particularly where there are signs of circulatory compromise or where colonoscopy has not identified a cause.
  • Cross-sectional imaging and angiography: CT angiography can localise active bleeding when it is brisk, and catheter angiography allows the bleeding vessel to be identified and, in some cases, treated in the same session.

An important consequence of this sequence is that identifying hemorrhoids does not by itself close the investigation. Hemorrhoids are extremely common, which means they are frequently present in people whose bleeding is coming from somewhere else entirely. Attributing bleeding to a visible hemorrhoid without considering whether the pattern actually fits is one of the recognized ways a more significant source is missed, and it is why age, systemic symptoms, and bleeding pattern continue to matter even when an obvious anorectal finding is present.

Treatment Depends Entirely on the Source

Because hematochezia is a finding rather than a diagnosis, there is no single treatment for it. What follows is directed at whatever is established as the cause, and the range is correspondingly wide — from addressing constipation, through endoscopic and catheter-based intervention, to surgery.

Where the source proves to be hemorrhoidal, treatment escalates in a fairly consistent order. Conservative measures aimed at reducing straining come first and resolve a considerable proportion of cases. Office-based procedures follow. Where Hemorrhoid Artery Embolization Fits

Hemorrhoid artery embolization is a catheter-based treatment for bleeding of hemorrhoidal origin. Access is gained through a small puncture at the wrist or groin, a catheter is advanced under imaging guidance to the superior rectal arteries supplying the internal hemorrhoidal cushions, and their flow is selectively reduced so the engorged tissue shrinks and bleeding diminishes. Because nothing is cut in the anal region, there is no wound in nerve-dense tissue to heal, which is the principal reason recovery differs so markedly from surgical excision. The limitation follows directly from the mechanism, and it is worth being precise about. Embolization addresses arterial inflow to internal hemorrhoidal tissue. It is therefore relevant only where hemorrhoidal bleeding has been established as the cause of the hematochezia — not as a treatment for the finding itself. It does not address diverticular bleeding, inflammatory or infectious causes, or any of the other entries on the causes list, and it is not a substitute for the diagnostic evaluation that establishes which of those is responsible. It is also not the appropriate treatment for an acute thrombosed external hemorrhoid, which involves tissue outside the territory it treats.

For appropriate candidates with confirmed hemorrhoidal bleeding that has not responded to conservative and office-based measures, embolization may offer meaningful improvement without a surgical wound, and it is of particular interest for people in whom surgery carries elevated risk — those on anticoagulation, with significant comorbidities, or who cannot manage extended recovery.
